How to submit Department of Health Abu Dhabi infectious disease notification

Please note that in order for notify the Department of Health Abu Dhabi about any infectious disease that you suspected or confirmed you must first have access to the Department of Health E services platform if you don't have an access please follow the other article under the subject of how to get access to the Department of Health infectious disease E notification system.

Step one open Google Chrome or Microsoft Edge type in Department of Health Abu Dhabi and select E services

Step two search for infectious disease notification or Click here to be redirected to the infectious disease notification portal page.

Step three log in with your uae pass and once you've locked in search for services in the top navigation bar locate infectious disease notification and then click on notification.

Step 4 Fill in the demography details of the patient along with that Scroll down assuring to add in the contact details of the suspected or positive case identified assure to select the appropriate type which is suspected or positive.

Step 5 go on to the next page and select what Disease are you notifying the Department of Health you may select more than 1 Type of communicable disease if you suspect or if it was positive for more than once please assure to select the appropriate communicable disease.

Step 6 go on to the third page assure to fill in the remaining information such as past medical history risk factors laboratory tests that was done and all the necessaries informations that were needed.

Step 7 double check your work once you are satisfied assure to click on submit wait for the infectious disease notification number once the number has been generated please print out the infectious disease form and add the infectious disease notification number in the patient's medical record (Infectious disease number starts with IDN).
