How to Submit a Death Notification

All deceased who died in the Emirates of Abu Dhabi death notification must be completed under the Department of Health Abu Dhabi Tamm services. Tamm could be access via web browser or mobile phoner, it is recommended when submitting the Death notification to submit it via laptop or PC and to use the recorded browsers Microsoft Edge or Google Chrome.

Steps of submitting the Death notification are:

1. Login to Tamm by using your UAE Pass

2. Search for Department of Health Abu Dhabi and find the service type Submit Death Notification.

3. Identify the type of Death if it is Submitted to Police Investigation or Natural Death.

4. Select if the Deceased  is known: If Yes (It means that the deceased was registered with a valid full name , ID, Date of Birth , Age, Gender etc..) if No then that means that the case was registered as unknown when arrived on the hospital ( Unknown patient are patient registered based on pre-register name who could not be identified, you should only select the category unknown when the deceased  was registered unknow)

5. For Known deceased assure to input the deceased information by first adding Emirates ID and DOB and validating the inputted information. If Emirates ID was not presented and you have the Passport of the deceased person you can chose manual and input the full information that matches the Deceased identification.

6. Please note that Tamm will require to input the Emirates ID and DOB of the  relative or anyone who knows about the deceased in the custodial details so that the morgue can contact the designated person to proceed with the next steps. If no one was present, you need to contact the Abu Dhabi Central Morgue to find a way to bypass this issue.

6. The Next step you will fill in the remaining details such as Date and Time of Death, Death cause type etc...

7. Once you reach the last step you will assure to choose the correct information about the morgue pickup number if you are in SKMC you may input this number (0097128196666) , morgue to be select in case of ( Police Investigation you will chose Central morgue, if non police case depending on the nearest and availability of the morgue or location to be set by family or relative member ( Keep in mind that if the morgue that was selected is busy they will send to the vacant morgue)

8. Add the physician details correctly like first and last name, mobile number and health care pretensioner  license (that starts with GD and not your hospital badge number.)

9. Prior to submitting the Death Notification assure all information are inputted correctly and once you double checked the information you may submit the death notification and assure that you take down the number that starts with DENO-2024 and added it to the patient medical record.
